The following section of the Benefit and Information Grid will be REMOVED:
COVID-19 Evaluation for testing, testing and related office visit, Teleheath, Urgent Care, and ER charges are paid 100% by the plan with no deductible, regardless of network restriction.,
Going forward, all COVID-19 evaluation and testing along with the related office visits, telehealth, urgent care, and Emergency Room charges will be payable under the normal plan provisions.
- COVID-19 immunizations will be covered under the PREVENTIVE/WELLNESS CARE benefit as follows: In Network 100% paid by Plan with no deductible, Out of Network paid after deductible then 40% paid by the Plan.
